We are happy to organise a nursing care consultation for our patients and their relatives for the period following their stay at the clinic. This is carried out in cooperation with the teams at the nursing stations in Hamburg-Mitte and Bergedorf. There you can discuss any questions you have in the period following your stay at the clinic.
Hamburg-Mitte and Bergedorf nursing stations
The teams at the nursing stations provide competent and independent advice and support to those in need of care and their relatives regarding all questions on the topic of care. This includes the following topics:
- Options for outpatient, inpatient or day-patient nursing care, 24-hour care
- Serviced living, alternative living arrangements or residential projects, residential care communities
- Hospice and palliative care
- Care insurance in accordance with SGB XI: Criteria for levels of care, application procedure, assessment, appeal procedure, entitlement to benefits, etc.
- Care costs and financing
- Billing for care services and clarification of invoices, if necessary
- Homes, care services, daytime care, etc.: specific offers in the different areas
- Health insurance in accordance with SGB V: Treatment care, home nursing, provision of assistive devices, travel costs, SAPV (specialised outpatient palliative care), hospice (outpatient/inpatient)
- Welfare benefits according to SGB XII: Help with care, domestic help, help for the elderly
- Care from one source; no referral consultation
- Specific support, e.g., for filling out applications
- Accompaniment to meetings, e.g., with service providers
- Individual and comprehensive care advice, i.e., help coordinating individual steps over a longer period of time (case management)
- Advice on power of attorney, healthcare proxy and living wills
- Care Management: Cooperation with other figures in the Hamburg aid network, networking, building networks
